Eligibility
Serves residents of Bedford, Bedford Heights, Bentleyville, Glenwillow, Maple Heights, Oakwood, Solon and Walton Hills. Exact boundaries may be determined by calling 211/First Call For Help. Service may be recieved once per month. Generally, income should be 200% of the Federal Poverty Limit or less. Households that do not meet the income guidelines but have extenuating circumstances (such as loss of income or some other financial hardship) may be served on a case by case basis.

Documents Required
Proof of Address/Residency: Current postmarked utility bill, Ohio Works First (OWF) statement, or other official document (such as that received from Social Security). Proof of Income: Public assistance benefit letter, Social Security benefit letter, Medicaid Card or paycheck stub. If income is over the guidelines, must bring proof of extraordinary medical expenses. Proof of Family Size: Birth certificates, Medicaid/Medical cards, or school enrollment forms (for each member of the household). Photo I.D. (For client AND his or her representative, if disabled.) However, individuals can be served once without documents. If income is over the guidelines, may be asked to bring proof of extraordinary extra monthly expenses.

Provides a variety of food services for those with HIV/AIDS. Provides two grocery bags of non-perishable food, food supplements, special fortified frozen meal delivery (5-day supply delivered each week), and 7 day home-delivered maintenance meals for the homebound (1 hot/1 cold -diabetic and low-sodium accommodated). Offers transportation for medical HIV-related appointments and nutrition counseling also.

Varies: days, evenings and weekends. Fortified frozen meals are delivered once per week. Hot/cold meals are delivered as often as 5-7 days per week, including weekends and holidays.

Those with HIV. For fortified frozen meal delivery, must be at risk of wasting; for hot/cold meals, must be homebound. Client can receive 2 bags of food once per month.
