Must have proof of residency (either lease, bill, or proof of homelessness- letter from shelter or family member). Must have late or eviction notice for rent assistance OR late or disconnect notice for utility assistance. Must have proof of household income and any assistance the household has such as (latest paystub, SSI/SSDI, TANF, unemployment, child support, food stamps, LIHEAP, etc), any assistance household has (food stamps, etc) Reason assistance is needed (medical bill, car repair, loss of wages, etc).
