CASE MANAGEMENT SERVIES: Long term case/care management for seniors Assistance is provided in the form of access to or care coordination in circumstances where the older person and/or their caregiver are experiencing diminished functioning capacities, personal conditions, or other characteristics which require the provision of services by formal service providers.

CONGREGATE MEALS: Congregate Meals provide a nutritious meal and reduces isolation of older persons by combining food with friendship and socialization The program is available to persons 60 years of age and over and their spouse regardless of age, contributions accepted

FARMER'S MARKET VOUCHERS: Provides Farmer's Market Vouchers to persons 60 and older

HOME DELIVERED MEALS: Provisions of a meal to an eligible participant at the client's place of residence which complies with the dietary guidelines for Americans

INFORMATION AND ASSISTANCE: Information and assistance is provided for all issues concerning aging

MEDICATION MANAGEMENT SERVICES: This program may include medication management, screening, and education It may consist of review of a person's medication to assess interaction and/or the setup of medication by a pharmacist or a nurse that results in assisting a person to remain at home.

NUTRITION EDUCATION: A program to promote better health by providing accurate and culturally sensitive nutrition, physical fitness or health information and instruction to participants and caregivers in a group or individual setting.

NEST PROGRAM: An incentive-based, parent education and support program to pregnant women and children through 3 years of age The goal of the program is to prevent birth defects and improve the health of pregnant women and their babies by providing incentives to encourage early prenatal care and parent education.
